Just checked, it's from season 4 ep 1 at 50:50, crazy looking
effects but I will follow the OP's wish and not spoil what happens
in it

It's at the end of the first episode of the fourth season. Male to
female, as OP clarifies. I can't really say any more than that
without huge spoilers for the episode and arguably the whole
series, so I'll leave it at that.
...except to say that From is an incredible horror series and you
should all watch it! Highly recommended.

The transformation's a bit graphic and apparently painful, with
bones visibly reshaping themselves beneath the character's skin.
For fans of that sort of transformation, it's really good.
The male character will likely remain in his female guise for much
of the show's fourth season, and I wouldn't be surprised to see
additional shapeshifting, so stay tuned.

The MtF character has been in every scene of the season so far, and
will likely continue to be in future episodes. There have been no
further on-screen transformations, though he does briefly return to
his male form in one episode (the transformation is off-screen).
