In 1985, Lee Grant directed a documentary for the HBO series
America Undercover. Titled "What Sex Am I?", it showcased the lives
of several transgender people, mostly post-op transsexuals, but
also some transvestites.
One of the individuals, profiled about midway through the show, was
a man who, at the (literal) last moment decided he didn't want to
go through with it. In his own words, "If they had started the
anesthesia 10 seconds later I would have told them, 'Stop, I don't
want this!' " Well, what a difference ten seconds makes. The guy
got only the first part of the surgery completed... the part
involving the most significant difference between men and women
(and I don't mean his subscription to Cosmo, either.) He sounded
like he was about to cry as he told Ms. Grant that he would have to
go through the rest of his life as a man with female genitals.

There was an episode that went exactly along these lines in the TV
series Lexx.
Some weird microbes transform Stanley and Xev, but only down where
it counts.

There is a ton of hentai that have women with penises, so we have
that covered, does anyone know of another kind of genital change in
a hentai? Like a man with a vagina, or some kind of animal change?
I would think, with how crazy hentai is, that there would be a
bunch, but I don't know of any.

DirkWilson - There was a manga (and anime) with the English title
of "After School Nightmare" whose main character was a high school
student who was "a boy from the waist up and a girl from the waist
down". Since that was simply how the character was and did not come
about by a transformation, I suppose it is at best only borderline
in terms of this conversation, but I thought I'd mention it.

It seems to me that the quickly-becoming-a-classic children's book
Bill's New Frock qualifies (or at least the ZTV adaptation does).
In the TV show Bill wakes up on that fateful morning, goes to the
bathroom, and discovers that there is something that boys can do
standing up that he must now sit down to do. And throughout the
book and the TV adaptation, even though Bill has to dress like a
girl and is treated like a girl, he still sees himself as he always
has before.
So at least the TV broadcast would qualify. And if the bathroom
scene is in the book (I would guess it is but I haven't read the
book so I don't know) then it would also qualify.

Some ten years ago a story appeared on some TG sites titled
Gynamorphisis. It was a first person account of a man who wakes up
to discover he has a vagina between his legs. Within a month, he
discovers that he has internal female organs as well.
The novella (which was possibly inspired by the Franz Kafka classic
story "Metamorphosis") details how the man gradually becomes more
and more feminine and finally accepts her new life as a woman.
