There's a few more instances of transformation (one male to male
possession, one female to male TF, and lots of humans to
monsters/creatures and vice-versa) in CDProjekt's expansions to
their Witcher 3 game, which was discussed a year ago here.
Spoilers ahead. Videos with nudity or blood will be tagged.
In the Hearts of Stone expansion:
- (Warning: Some toad gore earlier in the video)
The main character, Geralt, goes to kill a giant toad living in
some sewers. It's actually a prince cursed into being a killer
toad: https://youtu.be/A1iN6sd9cYI?t=2m45s
- Geralt is possessed by the spirit of a dead man's brother: https://youtu.be/CNoxHcDm0Ss?t=11m35s
In the Blood and Wine expansion:
- (Warning: Nudity and blood)
The first hostile vampire that Geralt fights is a Bruxa, which
disguises herself as a cloaked woman at a pub earlier in the story:

- (Warning: NSFW)
An old hermit meditating in the middle of a lake is revealed to be
the Lady of the Lake after Geralt beats him in combat: https://youtu.be/DPJ4n_1Gmzs?t=1m55s
- (Warning: Lots of blood and gore)
The main antagonist of the expansion, a vampire, transforms into
more horrifying, bestial forms as Geralt fights him at the end of
the story. Main parts are at 55 seconds, 3:25, and at 5:55: https://youtu.be/S-Td_aETp4I?t=55s
Can't remember what else was there. Some stuff is in the game's
flavor text.
